Health Connections Mendip helps patients access the local support and services they need to improve their health and wellbeing.
The project offers one-to-one support to patients in GP surgeries, holds a weekly talking café (Tuesdays at Peppers) in Shepton Mallet town centre and runs self-management courses for patients with long-term health conditions. The service is free and patients can self-refer.
